Your First Visit

Please arrive about 10 minutes early so that you have enough time to complete our New Patient and Medical History form. This form is designed to give us the information our team needs to provide you with personalized oral health care.

You can also download and print the form below to bring to your first appointment.

New Patient & Medical History Form

Initial Examination

Your dentist will begin by reviewing your medical and dental history and completing a comprehensive dental exam. Your exam will include a thorough examination of your gums, teeth, bite and jaw, an oral cancer screening and any X-rays that your dentist feels are necessary.

Treatment Plan

Based on your history and examination, your dentist will develop a long-term dental treatment plan for you. Your dentist will also take the time to answer any questions you have about your dental health and available treatment options.
